The list of live charities is curated and grows over time; there is no self-serve form. Listing a charity means a new config ID on donate.gg for it, the Safe adding that config ID to the approved list, and a new charity address on Robinhood Chain and receiver on Base deployed for it. The approved list exists because donate.gg's contract accepts any 32-byte config ID and strands ETH sent under a wrong one, so every listed config ID is vouched for before a charity address can exist (see Why permissionless).
